Cancel Ontario Health Card

To cancel an Ontario Health Card, Print off the Ministry of Health and Long-Term Care Change of Information form (click here) (The form is page 3)

Fill in Section A and Section F. Mail the form, the deceased person’s health card (cut into two pieces) and an original Proof of Death Certificate issued by the funeral home to the following mailing address:
